You must be currently attending, or have been admitted to, the University of Colorado School of Dental Medicine. To apply for the University of Colorado School of Dental Medicine Scholarship you must meet the general requirements of the scholarship, and include the following with your online application: a copy of the first two pages of your parent’s most current, completed federal tax return (IRS form 1040). If you are considered independent, you will need to submit only your tax forms., SAR (Student Aid Report), and one current letter of recommendation from persons not related to you.
