The Dr. Edward Jacobson Healthy Living Scholarship will be offered to students currently pursuing a program of study at an accredited community college, junior college, undergraduate or graduate institution in the United States. Those wishing to be considered for the award must submit proof of good standing from their educational institution of record as well as evidence of a current cumulative grade point average of at least 3.0 on a 4.0 scale.
