To apply for a University of Colorado School of Pharmacy Scholarship you must meet the general requirements, and include the following information with your online application:
A copy of the first two pages of your parent’s most current, completed federal tax return (IRS form 1040). If you are considered independent, you will need to submit only your tax forms. No W-2’s, SAR (Student Aid Report), and one current letter of recommendation from persons not related to you. See website for details.
