**multiple** An applicant must be a member of the Polish Union of America who complied with the requirements set by a rededication commitment letter sent in February 2011. Which means a member would have to transfer their previous policy, or purchase a new policy after January 1, 2011, or make a monitary donation to our scholarship fund. Please note that the donation must be made prior to the deadline in order to qualify for a scholarship. If you have any questions call: 716-677-0220.
