Applicants must be (1) dependents or spouses of an Oregon worker who has incurred permanent total disability (PTD) on the job while working for an Oregon employer OR (2) receiving, or have received, fatality benefits as dependents or spouses of an Oregon worker fatally injured on the job while working for an Oregon employer.
